OPEN POSITIONS
The role of a Direct Support Professional (DSP) is designed to help individuals with developmental disabilities,
mental health conditions, traumatic brain injuries, or chronic illnesses to increase independence. Direct
Support Professionals work one on one with a client in their own homes and in the community to build skills
that empower them, as well as help them to engage and integrate within their community.
Your role will look unique to each client as you come alongside them to take part in tasks such as running
essential errands, attending appointments, managing schedules, personal organization, household
management, financial management, meal planning, exploring, participating in hobbies, and more.
As a direct support professional, the rapport that you build with your clients will assist them in building the skills
and confidence needed to live independently in their community and achieve their goals.
Responsibilities:
● Meet with clients regularly as scheduled in their homes in addition to providing remote support
● Support clients through creative, thoughtful, problem-solving
● Ability to assist and execute clients’ goals and objectives
● Understand and work to follow through with support plan goals
● Assist clients in planning and structuring time together
● Timely completion of documentation of activities and progress as directed by supervisor
● Clear and professional communication with clients, supervisors, and members of the support team
● Transporting clients as needed throughout the community for appointments and activities
Qualifications:
● High School diploma or equivalent
● 2+ years of customer service experience
● Reliable car that is well maintained and suitable for client transportation
● Valid driver’s license and valid car insurance
● Ability to pass a criminal background check
● Ability to pass and maintain required training for DHS compliance such as CPR/First aid and HIPAA
Essential Functions:
● Occasionally move up to 20lbs for various client needs.
● Ability to remain in a stationary position for up to 75% of the session
● Frequently move within and about the clients’ residence and/or community, including
ascending/descending stairs or ladders.
● Have and maintain a valid driver's license
● Frequently, and throughout the session, verbally communicate with clients
● Ability to view clients in-person throughout sessions
The role of a Senior Direct Support Professional (SDSP) is a full-time position designed to help individuals with
developmental disabilities, mental health conditions, traumatic brain injuries, or chronic illnesses increase their
independence. An SDSP works one-on-one with clients, onboards new clients to assigned staff, trains new
staff with assistance from the Field Coordinator and helps transition clients between DSP assignments.
An SDSP will work a flexible schedule, based on the SDSPs predetermined availability, to ensure clients
receive appropriate care every week. The clients an SDSP works with will change regularly based on client
needs and the demands of the business. SDSPs will assist Field Coordinators to ensure appropriate coverage
and support for each client weekly.
As an SDSP, the rapport you build with your clients will assist them in building the skills and confidence needed
to live independently in their community, and achieve their goals. In addition to the client relationships SDSPs
build, SDSPs will also focus heavily on the development and onboarding of new Ehrnriter Cares staff.
The role of an SDSP is to be an ambassador for Ehrnriter Cares, exemplifying our core values through every
interaction.

● Support clients through creative, thoughtful, problem-solving
● Understand and work to follow through with support plan goals

● Timely completion of documentation of activities and progress as directed by supervisor
● Clear and professional communication with clients, supervisors, and members of the support team
● Transporting clients as needed throughout the community for appointments and activities

● Minimum availability of 30 hours per week
● Valid driver’s license and insured vehicle that is well maintained
● Ability to pass a criminal background check
● Ability to pass and maintain required training for DHS compliance such as CPR/First aid and HIPAA
● Strong preference for candidates with experience working with individuals with brain injury,
developmental disabilities, or severe and persistent mental health conditions.

The role of a Community Coordinator is a full-time position responsible for staff development and support, client engagement and transition, and ongoing neighborhood compliance. This position will report directly to the designated Field Coordinator and will work in partnership with the Agency Outreach team. This position includes both field and office work.
The Community Coordinator’s responsibilities include but may not be limited to:
Responsibilities:
Staff Development and Support
● Oversee field staff development and manage staff performance
● Onboard new clients and execute transition plans for assignment of field staff
● Develop skills and competencies in field team
● Develop high potential DSP Field Staff into Senior DSP and Community Coordinators
● Foster a culture of communication, engagement and recognition
● Be in consistent communication with Agency Outreach staff and Field Coordinators
● Respond to all emails within 24 business hours
● Lead by example via demonstrating behavior becoming of a Direct Support Professional
● Field Engagement
● Provide coaching and feedback to Field Staff on policy, operational processes, and client interaction opportunities observed through completion of compliance meetings
Manage Client Transactions
● Direct involvement and engagement with clients for all staff transitions
● Visit with clients as needed and provide remote support throughout staff transition
● Ensure each client receives service and communication weekly
● Work with Agency Outreach to coordinate and initiate services after intake
● Maintain case notes to assist with placements and client transitions
● Be considerate of the clients' time
● Execute client relations in a timely manner
Ongoing Neighborhood Compliance
● Partner with Agency Outreach Department to ensure timely and accurate completion of client paperwork
● Communicate with case management to notify of scheduled meetings and delivery of 245d paperwork
● Partner with Field Coordinator to complete internal quarterly client meetings
● Conduct work with a high level of efficiency
● Utilize work hours for related tasks
● Respond to text messages and calls from leadership within 2 hours
Neighborhood and Client Relations
● Meet with clients regularly as scheduled in their homes in addition to providing remote supports
● Onboard new clients and execute transition plans for assignment of field staff
● Support clients through creative and thoughtful problem-solving
● Willingness to learn from senior team members and adapt to evolving neighborhood needs
● Understand and work to follow through support plan goals
● Assist clients in planning and structuring time together
● Timely completion of documentation of activities and progress as directed by supervisor
● Clear and professional communication with clients, supervisor, and members of the support team
● Transporting clients as needed throughout the community for appointments and activities
Neighborhood Growth and Engagement
● Foster exceptional relationships with Case Management Agencies by engaging with case managers through excellent email and phone communication
● Assist with Neighborhood outreach in tandem with Agency Outreach by communicating with Case Management about emergent client needs and facilitate communication with Case Management and other County agencies
● Represent Ehrnriter Cares by embodying our mission and attitude during every encounter with Case Management, clients, and representatives from other County agencies
